IntroductionYounger age at diagnosis is a risk factor for poor health-related quality of life in long-term breast cancer survivors. However, few studies have specifically addressed HRQOL in young adults with breast cancer, nor have early changes in HRQOL been fully characterized.MethodsEligible female patients with breast cancer were identified through our local cancer center. To establish HRQOL, patients completed the Functional Assessment of Cancer Therapy-Breast around diagnosis and 12 months later. Sociodemographic factors, genetic susceptibility to cancer, tumor- and treatment-related factors, and (...) comorbidities were abstracted from medical records and the local oncology registry. Mixed-effects models were used to identify changes in FACT-B scores during the first year of treatment and to determine whether any demographic/treatment-related factors modulated changes in scores.ResultsHealth-related quality of life in young patients with breast cancer was within normal limits at baseline, with a FACT-B overall well-being score of 108.5. Participants reported slight improvements over a 12-month period: FACT-B overall well-being scores increased 6.6 points, functional well-being improved 3.0 points, emotional well-being improved 1.9 points, and physical well-being improved 1.5 points, on average. Participants with anxiety/depression at baseline reported greater improvements in FACT-B overall well-being and functional well-being than participants who did not have anxiety/depression at baseline. Marital status, reconstructive surgery, and baseline clinical staging were also significantly associated with changes in aspects of HRQOL, although their impact on change was relatively minimal.ConclusionYoung women with breast cancer do not report HRQOL concerns during the first year of treatment. Improvements in HRQOL during the first year of treatment may be attributable to a sense of relief that the cancer is being treated, which, in the short run, may outweigh the negative late effects of treatment. Context: The use of first-person micro-phenomenological interviews and their productive interaction with third-person physiological data is a challenging and pressing issue in order to offer an effective and fruitful application of Varela’s neurophenomenological hypothesis. Problem: We aim at offering a generative method of analysis of first-person micro-phenomenological interviews using third-person physiological data. Our challenge is to describe this generative first-person analysis with the third-person physiological framework rather than put Varela’s hypothesis into practice in a generative way (as we did in (...) another paper. Method: The present contribution is a first pioneering study as far as the exposition of such an interactive generative methodology is concerned. It is also a new issue insofar as it deals with a case study, surprise in depression, that has not been thoroughly dealt with so far, either in philosophy or in psychopathology. Results: We show that the analysis of first-person data is an intrinsic generative one, insofar as new refined categories and multifarious circular micro- and macro-processes were discovered in the very process of analyzing. They provide the initial structural generic third-person description of surprise inherited both from philosophical phenomenological a priori categories and from the experimental startle setting with a refined micro-segmentation of the dynamic of the experience. Implications: Our article could be of interest to neurophenomenologists looking for an effective application and to researchers in quest of a method of analysis of first-person data. The present limitations are due to the still preliminary data-results we need to complete. On various principles of economics in an Islamic society.

The extremely successful _Standard Model of Particle Physics_ allows one to define the so-called _Elementary Particles_. From another point of view, how can we think of them? What kind of a status can be attributed to Elementary Particles and their associated quantised fields? Beyond the unprecedented efficiency and reach of quantum field theories, the current paper attempts at understanding the nature of what these theories describe, the enigmatic reality of the quantum world.

The Oregon Basic Health Services Act of 1989 seeks to establish universal access to basic medical care for all currently uninsured Oregon residents. To control the increasing cost of medical care, the Oregon plan will restrict funding according to a priority list of medical interventions. The basic level of medical care provided to residents with incomes below the federal poverty line will vary according to the funds made available by the Oregon legislature. A rationing plan such as Oregon's which potentially (...) excludes medically necessary procedures from the basic level of health care may be just, for the right to publically-sponsored medical care is restricted by opposing rights of private property. However, the moral acceptability of the Oregon plan cannot be determined in the absence of knowing the level of resources to be provided. Finally, Oregon to date has failed to include the individuals being rationed in discussions as to how the scarce resources are to be distributed. (shrink)

Because of cultural differences between East and West, any attempt at outright adaptation of Western ideas in Asia will undoubtly encounter problems, if not rejection. Transferring an idea from one place to another is just like transplanting an organ from a donor to a recipient—rejection is to be expected. Human cultures respond to new ideas from different value systems in very much the same way.Recently, biomedical ethics has received much attention in Asia. Fundamental advances in medicine have motivated medical scientists (...) to look at the ethical issues arising from this progress. Will the principles upheld by the bioethicists in the West meet the challenge in Asia? This article argues that Asian bioethicists must develop a bioethics responding to their own cultural contexts. If Western principles are adopted, then they must be re-interpreted and even modified, if necessary, in light of Asian beliefs. (shrink)

We show that the divergence between the predictions of quantum optics and the local realist theory known as stochastic optics, for the extended type of photon-coincidence experiment described recently by de Caro, is of the same order of magnitude as for Aspect-type experiments. This means that, in such new experiments, as in those so far performed, counting statistics will have to be greatly improved before a discrimination between the two theories becomes possible.We also show that the outstanding difference between the (...) two theories is that, while stochastic optics uses a genuine, that is, positive, detection probability, the corresponding quantum formalism leads to a pseudoprobability. Nevertheless, there is a striking parallel, in that both theories recognize the phenomenon known as enhancement, and both describe it as having its origin in a mixing of the signal field with the zero-point field by a polarizing device. Interactive narratives are inextricable from the way that we understand our encounters with digital technology. This is based upon the way that these encounters are processually formed into a narrative of episodic events, arranged and re-arranged by various levels of agency. After describing past research conducted at the iCinema Research Centre at the University of New South Wales, this paper sets out a framework within which to build a relational theory of interactive narrative formation, outlining future research in the area.

This paper takes the case of the proposed building of the Traveston dam on the Mary River in Australia to examine the ways postcolonial power relations are played out in city/regional relationships to further the interests of the city. Postcolonialism is concerned with unravelling multiple histories of colonialism, and identifying the reproduction, contestation, ambivalence and transformation of modes of domination and subordination in colonial relations. Political contingencies and contestations by residents, farmers, traditional Indigenous owners and environmentalists seeking to protect endangered (...) species are examined to highlight the dominance of Western anthropocentric, technocentric, and eurocentric epistemologies in political, ecological, environmental, biotheical debates. Il réunit des professeurs et des doctorants africains et non africains ainsi que des professionnels et des représentants de la société civile autour de questions de justice sociale et de politique publique. À l'initiative de doctorants africains et non africains, il a été mis en place conjointement par la Chaire Hoover de l'Université catholique de Louvain et par le Centre d’études et (...) de recherches sur la justice sociale et politique de l’Université catholique d’Afrique centrale dans le but de bâtir une véritable solidarité entre le Nord et le Sud dans le domaine de la formation académique et de la recherche. The Yaoundé PhD Seminar-Theories of Justice is an international and multidisciplinary seminar which is held since 2012 in Yaoundé. It brings together teachers and African and non-African doctoral students as well as professionals and representatives of civil society around issues of social justice and public policy. At the initiative of African and non-African students, it has been implemented jointly by the Hoover Chair of the Catholic University of Louvain and the Centre of studies and research on social justice and political of the Catholic University of Central Africa in order to build a genuine solidarity between the North and the South in the field of academic training and research.
